State illness assistance fund All state govts./Ut administrations have been advised vide ministry of health & family welfare letter dated 11/11/96 to set up an illness assistance fund in their respective states/uts. It has been decided that grant-in-aid from central government would be released to each of these states/uts (with legislature) where such funds are set up. The grant-in-aid to states/uts would be to the extent of 50% of the contributions made by the state govts./Uts to the state fund/society subject to a maximum of rs. 5 Crores to states with larger number and percentage of population below poverty line viz. Andhra pradesh, Bihar, Madhya Pradesh, Karnataka, Maharashtra, Orissa, Rajasthan, Tamil Nadu,Uttar Pradesh and West Bengal and Rs. 2 Crores to other states/uts. The state/ut level funds could also receive contributions/donations from donors, as mentioned for ran. The illness assistance fund at the state/ut level would release financial assistance to patients living in their respective states/ut up to rs. 1.5 Lakh in an individual case and forward all such cases to ran, where the quantum of financial assistance is likely to exceed rs. 1.5 Lakh. The following states/uts (with legislature) have set up illness assistance fund:- Karnataka, Madhya Pradesh, Tripura, Andhra Pradesh, Tamil Nadu, Himachal Pradesh, Jammu & Kashmir, Maharashtra, West Bengal, Kerala, Mizoram, Rajasthan, Goa, Gujarat, Sikkim, Bihar, Chhatisgarh, Jharkhand, Haryana, Uttarakhand , Punjab and Uttar Pradesh and the act of Delhi and Puducherry. The following states/uts have not yet set up the states illness assistance fund in spite of repeated reminders: 1. Assam 2. Manipur 3. Arunachal pradesh 4. Meghalaya 5. Orissa 6. Nagaland it has also been provided in the scheme that union territories (which do not have legislature) will be sanctioned a budget outlay out of the niaf, as and when the ut administrations set up illness assistance society/committee. It was decided in the first meeting of the managing committee held on 21-10-98 that each ut will be sanctioned a budget outlay of rs. 50 Lakhs. Accordingly the following uts have been sanctioned a budget outlay of rs.50 Lakhs each, during the year1998-99. 1. Lakshadweep 2. Daman & diu 3. Dadra & nagar haveli 4. Andaman & nicobar islands 5. Chandigarh

    Amount: From Rs.25,000/- to maximum Rs.2,00,000/- States/UTs (with Legislature) have set up Illness Assistance Fund that offers coverage up to Rs. 1 Lakh for cancer treatment at government hospitals within the state. While some states do not have this scheme, Karnataka, Madhya Pradesh, Tripura, Andhra Pradesh, Tamil Nadu, Himachal Pradesh, Jammu & Kashmir, Maharashtra, West Bengal, Kerala, Mizoram, Rajasthan, Goa, Gujarat, Sikkim, Bihar, Chhatisgarh, Jharkhand, Haryana, Uttarakhand , Punjab and Uttar Pradesh and the NCT of Delhi and Puducherry support it. Eligibility: Check if your state offers SIAF Download the application form Submit with BPL card and two photographs at the government hospital. Eligibility for Assistance under SIAF Only for persons below the poverty line suffering from specified,lifethreatening disease. Assistance admissible for treatment in Government Hospital only. Central Government/State Government/P.SU employees not eligible. Re-imbursement of medical expenditure already incurred not permissible.However, in exceptional cases, reimbursement could be allowed on acase to case basis with due approval of the Management Committee, provided the eligible patient has already applied for financial assistance before taking medical treatment/operation under emergency situation only and paid the dues to the concerned Hospital/Institute. Diseases of common nature and disease for which treatment is available free of cost under other health programmes/schemes not eligible for grant. Patient taking treatment in his/her State has to avail assistance from State Illness Fund (where such fund has been set up) provided medical estimate does not exceed Rs.1.50 lakh. Cases of estimates above Rs.1.50 lakh to be referred by States for assistance from RashtriyaArogyaNidhi (Central Fund) by the Ministry of Health & Family Welfare. ELIGIBILITY & PROCEDURE The person should be the domicile of the state in which they are appliying, and Belong to Below Poverty Line, and There is a standard form available at following places Office of the District Collector Office of District Chief Medical and Health Officer Office of the District Civil Surgeon District Hospital The applicant has to give application in prescribed from to Collector of the District in the name of Director Public Health & FW , The Collector / Sub Divisional Magistrate will give certificate of below poverty line & residential certificate and forward it to Civil Surgeon for certification of the disease, The estimate of the recognized institution (Economy class ) shall be attached for giving grant . the application is forwarded to the Secretary, State Illness Assistance Fund . The application is scrutinized and if found complete & fit within the ambit of SIAF the application is examined by a Sub committee. The management constituted under chairmanship of Honorable Minister, Public Health & Family Welfare gives the final sanction for the case
